To order copies of birth, marriage or death certificates online, simply click on the link below: The Town Clerk’s Office has vital records for the Town from 1640 to the present. To request a birth, death, or marriage certificateby mail, click on the following link: Request for Vital Record. Complete the appropriate section on the form and mail together with a check for $10.00 for each certificate requested to the address listed above. Certified Birth Records The town will have the birth record of any person whose parents resided in Braintree at the time of the child's birth. Long Form ($10.00): contains all information at time of birth (parents, place of birth, physician, etc.) The long form is needed for passports, insurance purposes, etc. Short Form ($5.00): shows name, date and place of birth. The short form is generally used for routine purposes such as school registration, sports sign-ups, work employment. Certified Marriage Records Certified Death Records The town will have the death record for any person who passed away in that town or was a resident of the town at time of death. |
